Transcript Requests may be submitted in writing and by either written letter, in-person, or fax. When requesting a transcript by mail or fax, students should let us understand:
* your full name, social security number and CASC ID (if known), last date attended, and any former names
* where to mail the transcript (name & address of college/agency/other)
* when to mail the transcript
* signed by student.
Students may order a transcript by
downloading a Transcript Request form and deliver the request to the:
